Filling in the questionnaire

In some cases, you will be sent a questionnaire following an appointment, before or after an operation or admission or during your treatment. For your safety, it is important that you answer all questions honestly. Only then will a responsible decision about your treatment be possible.

If you are not yet using mynexuhealth but the hospital has your e-mail address, we will ask you via e-mail to answer the necessary questions.
