Learning from incidents

A patient safety incident is an unexpected event in the care process that has led, could have led, or could still lead to harm to the patient. One of the cornerstones of UZ Leuven’s quality policy is to learn from these incidents, enabling us to implement improvement actions to prevent similar occurrences in the future. To achieve this, UZ Leuven has a system in place for reporting and analysing incidents.

We strive to turn every incident into a learning experience, with the aim of ensuring even safer care. To make this possible, we invest in openly addressing incidents in a constructive, solution-oriented manner. We take various initiatives in this respect, such as discussing incidents during Mortality & Morbidity Meetings. These are team meetings where an unexpected event or case is examined in depth. In this way, we foster an atmosphere in which staff can speak freely about what happened, and we encourage them to reflect on what could be done differently or better.

Improvement actions taken in response to an incident are sustainably embedded in our way of working, in order to enhance patient safety.

If you were involved in an incident as a patient, or if something happened that could have gone differently, you can report it via kwaliteitsbeleid@uzleuven.be. By doing so, you help us make our care even safer.
