Informed consent for day admission or general anaesthesia

On this webpage and in this PDF document, you will find more information about your informed consent and the necessary guidelines for a smooth process. It is important that you read this information before your admission to the day hospital. We therefore ask you to read, print, and sign the PDF document at home and bring it with you on the day of your admission. You can hand it over to the nurse preparing you for the procedure. If you do not have a printer at home, you can still sign the document at the day hospital.

If you are coming in for a day admission or have undergone general anaesthesia, you may only leave for home if:
- you have arranged for accompanied transport. You are not allowed to drive yourself (car, bicycle, etc.);
- you remain under the supervision of an adult for the first 24 hours after discharge, including overnight.
If this is not possible, it may be decided to postpone the procedure or that you must stay for one night.

Welcome to Dagziekenhuis 4
You will soon undergo a treatment or procedure via Dagziekenhuis 4. The pathway you will follow after the procedure will always be communicated to you in advance.
There are three possible scenarios:
- After your procedure, you return to Dagziekenhuis 4 and can go home the same day (day admission).
- After your procedure, you stay for one night in Hospitalisatie 36 (overnight stay).
- You stay longer in a hospital ward.

Choose Parking Oost, which is the closest to the day hospital. Follow the signs for "P Oost" on the ring road around the Gasthuisberg campus.

Patients can be dropped off and picked up at the kiss-and-ride zone at entrance Toegang Oost. Please note that stopping time there is limited. If your companion is accompanying you all the way to the day hospital, please use Parking Oost.

To make things easier for you, we have summarised the key points in a concise checklist that you can print yourself to ensure you are well prepared for your visit to Dagziekenhuis 4.
Anaesthesia
Before any procedure or treatment under general or local anaesthesia, an assessment must be carried out by the anaesthetist. This takes place during the preoperative anaesthesia consultation.

During the consultation, you will receive information about the type of anaesthesia, possible side effects, and any medication you need to take or stop before the procedure. You will also be given guidelines on fasting before the operation.
In some cases, a pre-anaesthesia consultation is not required. Instead, you will need to complete a preoperative file and return it to the preoperative anaesthesia consultation. If this applies to you (you will be informed during your consultation with the surgeon), you must send the preoperative file back no later than one week before your scheduled procedure or examination. This can be done by post, email, or fax. You can find the anaesthesia department's contact details above.
Hygiene
You must wash yourself thoroughly the evening before or on the day of the procedure. Your doctor will provide the correct guidelines.
Your doctor will also inform you whether you are allowed to shave the area to be operated on. In some cases, you should not do this yourself to avoid local skin infections. If necessary, hair removal will be carried out at the day hospital.

What time should you arrive?
At Dagziekenhuis 4, we aim to keep waiting times as short as possible. That’s why a member of our planning team will contact you two days before your procedure to inform you of the exact admission time.
If your procedure is scheduled for Monday, you will be contacted on the Friday before.
What should you bring?
Please read the instructions carefully via the link below. For admission to Dagziekenhuis 4, we ask that you bring only limited belongings (for one day). If you need to stay in the hospital for a longer period, we ask that your luggage be brought later to the hospitalisation ward where you are staying. Please leave money and valuables at home.

Who will accompany you?
What if you are discharged on the same day?
- You must be accompanied by one adult.
- For patients under 18 years old, both parents may be present. Siblings or other children must stay at home.
- The companion may wait in the room during the procedure but is not required to stay. They may also go to the visitors' cafeteria or coffee bar at the Gasthuisberg campus, or return home temporarily.
- The companion must pick up the patient from Dagziekenhuis 4 after the procedure.
- It is recommended to bring a wheelchair to facilitate the transport of the patient after surgery. Wheelchairs are available in the entrance hall of the Toegang Oost entrance.
- Important: Dagziekenhuis 4 closes at 20:00. Please pick up the patient by 19:40 at the latest.
What if you are admitted overnight to Hospitalisatie 36?
- You must be accompanied by one adult to avoid unnecessary crowding and ensure a calm environment for patients.
- For patients under 18 years old, both parents may be present. Siblings or other children must stay at home.
- Once the patient is taken to the operating room, the companion may return home.
- If the companion wishes to visit the patient in the overnight stay unit (Hospitalisatie 36), they may do so during visiting hours from 16:00 to 20:00.

Registration and check-in
On the day of your procedure, report directly to the reception of Dagziekenhuis 4 at the agreed time, unless you have prior appointments in another department.
Do not check in first at the registration desk at the Toegang Oost entrance.
- Our staff will go through the administrative process with you and provide any additional information if needed.
- You will receive an identification bracelet, which you must wear throughout your stay.
- Then, take a seat in the waiting room.
- You will be called via the screen and directed to door 4, where a nurse will come to collect you.
- Please note that patients are not called in order of arrival.
- Eating or drinking is not allowed in the waiting area. If your companion wishes to eat, they can do so in the visitors' cafeteria De Vier or the coffee bar.
Prefer not to have your photo displayed on the call screen?

By default, the call screen displays your queue number along with your photo. If you would prefer not to have your picture displayed, please inform the reception staff at the day hospital during check-in. If you use mynexuzhealth, you can also disable the display of your photo (via the ‘Profile’ menu).
Preparation for Your Procedure
- The nurse will take you from the waiting area to your room and check your identity, operation site, blood pressure, pulse, medication, weight, and other relevant details.
- You will be given a surgical gown.
- If necessary, the surgeon will mark the surgical site.
- The nurse may also perform additional procedures such as drawing blood, placing an IV, collecting a urine sample, taking a swab, or administering a sedative.
- The nurse will inform you when it is time to go to the operating theatre or the IRCC.
Personal belongings
- If you are staying for a day admission, your personal belongings can be safely stored in the cupboard in your room. However, space is limited.
- If you are staying one or more nights in a hospital ward, your luggage will be taken with you.
In both cases, we recommend you only bring essential items.
During and immediately after your procedure
Depending on the type of procedure, you will be treated either in the operating theatre or in the IRCC.
In the operating theatre
During the procedure, you will receive extra oxygen through a mask, and sleep and pain medication will be administered through an IV. The anaesthetist will monitor you throughout the operation.
At the IRCC
Depending on the procedure, you will receive either local or general anaesthesia. More information about the different procedures is available on the IRCC welcome page. If you received local anaesthesia, you will be taken back to your room at Dagziekenhuis 4 after the procedure.
In the recovery room (only after general anaesthesia)
If you underwent general anaesthesia, you will first be taken to the recovery room. You will stay there until you are fully awake and any pain or nausea is well controlled.
If everything is in order:
- for day admission, you will return to Dagziekenhuis 4.
- if you are staying one or more nights, you will be transferred from the recovery room to your hospital ward.
Post-operative care at Dagziekenhuis 4 (only for day admission)
If you are in day admission, you will be monitored at Dagziekenhuis 4 until you are ready for discharge.
- Your vital signs will be monitored, and if necessary, additional medication will be given for pain or nausea.
- Depending on the procedure, you may be given something to eat and drink.
- The recovery time varies from one to six hours, depending on the procedure.
- The doctor and nurse will give you instructions on pain management and wound care at home. Follow these carefully.
- You will also receive your prescriptions, certificates, follow-up appointments, and a letter for your GP.

Going home after a day admission
- You may leave Dagziekenhuis 4 after permission from the doctors and when you feel sufficiently recovered.
- Please note that your companion must pick you up by 19:40.

Important conditions for going home

You may only go home under supervision.

You must remain under the supervision of an adult for the first 24 hours after discharge.

You are not allowed to drive a vehicle (including a bicycle) for the first 24 hours.

The consumption of alcohol is prohibited for the first 24 hours.
